Design the PWL form layout, then publish it for use.
Create the template first in PWL templates.
Open the PWL templates list.
Select the template.
Select Design Template (or Actions → Design Template).
Designer controls match ITR templates (Default, Favourites, Insert, Cancel).

Button Group
Single Line Text
Text Fields with Labels
Radio Buttons / Checkboxes
Table
Text Box
Date Picker
Image
Use Preview to verify the result.
Select Save as Draft to save without activating.
Select Save & Publish to publish.
Use Actions to manage templates:
Edit
